The Sponsors of this Contest are the Bethpage Medical Staff Association and St. Joseph Hospital, a not-for-profit organization located at 4295 Hempstead Turnpike, Bethpage, NY.

The purpose of the Contest is to engage amateur photography enthusiasts in submitting photographs that may be used to beautify the hospital for the enjoyment of patients, visitors and staff alike.


ENTRY DEADLINE
All entries must be submitted by September 29, 2017 at 11:59 pm.  Proof of submission is not proof of receipt.


WHO MAY ENTER?
The Contest is open to amateur photographers only.
 
HOW TO ENTER
Each entry consists of an entry form, a single image and an entry fee.  The entry fees are as follows:  $10 entry fee per photo; $5 discount for students; $5 discount for CHS employees; $40 group discount (5 entries)
To enter, visit www.StJosephHospital.CHSLI.org and go to “click to submit photos” and follow the instructions. There is no limit to the number of entries per person.
 
The Categories for entries are: (1) Nature & Landscapes (Please include the name of the place in the title or description of your entry.) (2) Animals, (3) People, (4) Flowers, (5) Black and White, and (6) Catch-all. The Catch-all category includes everything and anything a camera captures, or even something entirely created in editing softwares.
 
Photographs must not contain obscene, defamatory, sexually explicit or otherwise objectionable content, to be determined by the discretion of the Sponsors.  All such photographs will be disqualified.
 
Photographs containing watermarks will be disqualified.
 
Photographs must be in digital format and of high resolution.
 
Photographs must not have previously been entered or have won an award in a prior St Joseph Hospital Photo Contest.
 
Photographs that include images of people, sculptures, statues, paintings and other works of art will be accepted as long as they do not constitute copyright infringement or fraud.  All Entrants must be prepared to provide a release form described below.
 
All photographs must be original material taken by the Contest Entrant.  By entering the Contest, the Entrant represents, acknowledges, and warrants that (a) he/she is 13 years of age or older, (b) the submitted photograph is an original work created solely by the Entrant, (c) the photograph does not infringe on the copyrights, trademarks, moral rights, rights of privacy/publicity, or the intellectual property rights of any person or entity, and (d) no other party has any right, title, claim, or interest in the photograph.
 
RELEASES
If the photograph contains any material or elements that are not owned by Entrant and which are subject to the rights of third parties, or if any persons appear in the photograph, the Entrant is responsible for obtaining, prior to the submission of the photograph, any and all releases and consents necessary to permit the use of the photograph in the manner set forth in these Official Rules without additional compensation or approval.  If any person in the photograph is under the age of 18, the signature of a parent or legal guardian is required on each release.
 
Upon the Sponsors request, each Entrant must be prepared to provide a signed written license from the copyright owner of any sculpture, artwork or other copyrighted material that appears in the photograph entry, authorizing any party to reproduce, distribute, display the entry in connection with the Contest and the promotion of the Contest in any media or format now or hereafter know.  Failure to provide such releases may result in disqualification and selection of an alternate winner.
 
AWARDS
The First Place winner in each Category will win $150.
The Second Place winner in each Category will win $100.
The Third Place winner in each Category will win $50.
A Best of Show winner will be selected from all of the entries; prize to be determined.    
 
All winning Entrants will be invited to a reception on November 9, 2017 at 6:30 p.m. in the Hospital’s Staff Room.  Winners of each category will be announced at the reception.
 
Winning photographs will be enlarged and framed for display in the common areas and patient rooms of St Joseph Hospital.

Potential winners shall be required to sign an affidavit, declaration of eligibility and liability and publicity release in which Entrant warrants that he/she is the creator and owner of the photograph (and all intellectual property rights in the photographs submitted) and grants Sponsor the right to reproduce, distribute, and display the photograph (with a name credit) in connection with the Contest and promotion of the Contest without additional compensation.  Failure to execute and deliver any required documents to Sponsor by a specified deadline may result in disqualification from the Contest and selection of an alternate potential winner.
 
JUDGING
Photographs will be evaluated by a panel of judges based on the following criteria: (1) creativity/originality, (2) technical excellence, (3) composition (4) artistic merit and (5) overall impact.

The entries in each Category will be judged separately.  All decisions of the judges are final. 

Winners will be selected in early October and notified by email.  
 
LIMITATION OF LIABILITY
By entering this Contest, Entrant agrees to release, discharge and hold harmless Sponsors and its partners, affiliates, officers, agents, employees, representatives and volunteers from any claims, losses and damages arising out of his/her participation in the Contest or Contest related activity.

Sponsor assumes no responsibility for any error, omission, interruption or deletion, defect, or delay in the operation or transmission; communications line failure, theft or destruction of or unauthorized access to the Contest entries or entry forms,  Sponsor is not responsible for any problems with or technical malfunctions of the web-based Contest entry program or the failure of any submitted entry to be received on account of technical problems or traffic congestion on the Internet or any website, human errors of any kind, or any combination thereof, including any injury or damage to Entrant’s computers related to or resulting from participation and the uploading or downloading of any material related to this Contest.
 
CONDITIONS
Entrants agree that this contest shall be governed by the laws of New York disregarding any conflict of law provisions.

Entries shall be void if Sponsors determine the photograph is not an original, or if the entry is incomplete, damaged, altered or obtained through fraud or theft. Sponsors reserve the right to verify the validity and originality of any entry and/or Entrant (identity and address) and to disqualify any Entrant who submits an entry that is not in accordance with these Official Rules or who tampers with the entry process.  Failure by Sponsors to enforce any of its rights at any stage does not constitute a waiver of these rights.

By entering this Contest, Entrant further agrees (a) to be bound by these Official Rules, (b) that the decisions of the judges are final, and (c) all winning photographs, and the winner’s name and likeness may be used by the Sponsor in any publicity relating to the Contest or future Contests and the winning photograph may be displayed at the Hospital without additional compensation or approval.  All federal, state and local taxes on prizes are the sole responsibility of the prize winners.  In the event, a winner refuses the prize or refuses to execute the affidavit, declaration of eligibility and liability and publicity release, the prize will be forfeited.  In its sole discretion, Sponsor may choose whether to award the prize to another Entrant.
